Case referral forms
Each client - even pro bono clients - should sign a representation agreement (and such agreements are required by our malpractice carrier). The agreement below is a sample agreement; feel free to tailor it to your individual needs.
Pro bono reporting forms
As part of our annual reporting, Heartland submits data about the number of cases and hours contributed by our volunteers during the last year. In addition, Indiana's Rule 6.7 now requires all of its attorneys to report the total number of pro bono hours contributed as part of the annual registration process. You can submit your hours using the form below, or update your case on Heartland's pro bono database (please email the plan administrator to get a login).

filing a pro bono case
If you represent a client in a civil action or a petition to appoint a guardian pursuant to IC 29-3-5, and you accepted the case as a direct pro bono referral from Heartland, your client is entitled to have his or her filing fee waived under IC 33-37-3-2(b). To receive this waiver, the attorney must first file a statement that complies with the requirements of IC 33-37-3-2(b).

Currently, Heartland does not have funds available to reimburse attorneys for litigation expenses incurred in the course of pro bono representation. If you would like to contribute to a litigation expense fund, please contact the Plan Administrator.
